  • Clinic card (5 year inoculation) / Kliniekkaart (5-jaar inspuiting)
  • Report from previous school (Gr 2-7) / Rapport van vorige skool (Gr 2-7)
  • Report from previous school or Little Elephants (Gr R) / Rapport van Gr R of Little Elephants
  • Proof of residence / Bewys van woonadres
  • Photo copy of ID: (both parents) / Fotostaat van ID dokumnet (beide ouers)
